CARE FOR YOUNG WOMEN
Prepare the
young women in partnership with the placing authority for their return
to an open situation, including their families and communities. To act at
all times in accordance with the principles of national legislation and
local policies relating to childcare. Education
An assessment
of the individual education needs of the young women admitted to St. Catherine's
forms part of the planning process. Each young woman will have an Individual
Education Plan as great emphasis is placed on the individual and her needs. HEALTHCARE At St. Catherine's we take a holistic approach to health with an emphasis placed upon the development of a healthy lifestyle as opposed to focusing just upon health problems. Each young woman will be registered with a local GP. St. Catherine's also contract the services of a part-time Nurse via St. Helens Primary Care Trust. A health assessment is carried out by the on-site Nurse and a healthcare plan is drawn up in order to meet the young woman's needs and record details of input. This is monitored by the on-site Nurse and Key Worker and the young woman is provided with a copy of the health plan. In the case of pregnant young women, appropriate ante-natal and post-natal care is accessed and overseen by the site Nurse. Dental and ophthalmic appointments are made as appropriate. St. Catherine's
have a contract with the 5 Boroughs Partnership NHS Trust. The Community
Homes Health Team (CHHT) provides St. Catherine's with an extensive range
of health services, including assessment, intervention and a transfer
package for each Young Person, with reports at each stage.
